Overheating/Radiator Flow Problem?

Car has been overheating recently and I haven't had time to mess with it, though I should have. For the last week every day my car would start overheating, and the reservoir would be empty. I would fill it back up, drive it, and that night it would be empty again. Finally I got a chance to look at it, and after driving it for 2 minutes from a cold engine it started overheating. Reservoir was full before I took off, and when I got back it was completely empty. The upper radiator hose was rock hard, and at the fitting where it goes into the engine there was a small hole that was slowly letting pressure out. Is there a blockage or is it that valve (cant remember what its called) that opens to let the radiator fluid flow in that is malfunctioning?
