I have left lean bank code that comes and go. We have smoked the engine, replaced the MAf all the 02 sensors there are no other codes, This the second engine with the code, We also removed the intake check fuel pressure and codes. There are some problems with fuel trim as well, I am at the end of my rope

When diagnosing a problem like this there are a few questions that we need answers to.
When is it lean?
At idle only?
Include changes based on no load engine speed, less than 800 rpm, 1000rpm, 1500rpm, 2000rpm.
At cruising speeds only? Again, does the engine speed reveal any changes?
At heavy acceleration only?
OR
All the time (idle, cruise and acceleration)?
OR
Any engine load or speed but not all of the time?
Is only one bank lean (adding fuel) and the other bank rich (taking away fuel)?
What is the fuel pressure?
How much alcohol is in the fuel?
Have you done a volumetric efficiency comparison of the MAF and is the actual airflow through the engine being reported?
Do all four O2 sensors agree to changes in fuel control?
That's a start and what I would be getting answers to in order to choose my next step.
