Payment based on 60% residual value and 0.00270 money factor.
The lease looks as follows: 36 months @ 12 miles per year
$5,000 total due at signing
$638 per month including sales tax and all fees.
MA tax is 6.25%
The total out the door price for purchase is below:
$43,785.00................MSRP
$2,774.06..................Sales tax
$730.00.....................Registration, License, Title and Documentation fees
$47,289.06................Total price out the door
What am I missing?

You only have the purchase numbers, instead of the actual numbers for a lease.
But, making educated guesses, the $5000 DAS would approximately cover:
$730 fees
$925 lease acquisition fee
$638 1st payment
$2500 CAP cost reduction
$200 upfront tax on CAP reduction/acq.fee.
$43785 selling price
-$2500 CAP reduction
= $41285 Adjusted CAP
$599/mo + tax = $636/mo (that's pretty close)
No discount from MSRP plus a .00050 markup to the money factor will never result in a great payment.
