While you wait for someone thats might actually know what it may be, I might wonder about stuck open PCV valve or something like that (if so equipped)

I have a PCV hose but I do not see a valve.

It would be on valve cover side. How does that air filter look BTW?

The air filter looked okay, I left it out and and made sure everything from the air filter to the Throttle body was correct and tight, and the problem stopped. I then installed a new air filter and the problem returned.

can you hear any louder exhaust noise than before ,consistent with cracked exhaust manifold? i'm 84% sure that you can safely do a test run safely with the PVC hose blocked with a pencil or whatever is right size to block it. also consider check the charcoal/evap system, could there be a loose/cracked hose there actually the cause? have you looked for any stored/silent OBD codes?
