Sudden unintended acceleration

We had an incident of sudden unintended acceleration while pulling into a parking lot. Pulled in at a slow, reasonable speed, vehicle was in drive, and then the vehicle took off. Foot was on the brake. No issues with mats. Please let me know if you have any thoughts about things you would have looked at. ECV is apparently normal according to dealership. Also, are there any reports that should be filled out in Ontario, Canada for this sort of thing (other than Transport Canada)? What would you do if it were your vehicle?
