Radiator fan not working

2006 Hyundai Sonata radiator fan does not come on at temp. or with AC. Fan comes on when jumped with 12 volts and a good ground. Wire to fan has 14.5 volts on positive side and 12.5 volts on the negative side when unplugged from fan and car running. Why is there voltage on the negative side and how do I fix?
