April 19, 2017 I was pulling in to a parking space at a grocery with my foot on the brake and the car accelerated at a high rate. I had to bury the brake with both feet, put it in park and turn off the engine to get it to stop. 2007 4Runner V-8. Second time this has happened in last 6 months. First time it happened I was pulling in to garage and car almost went through back of garage. Exact same circumstances as first time. Foot was on brake slowing down and car started to excel erase at high rate of speed.

First check the floor mat, make sure it isn't on top of the throttle.

Otherwise, the most likely cause is that you are catching the throttle with your right foot as you go to brake. The throttle goes to the floor before you can push the brake pedal very far which can give you that engine racing and no-stopping condition.
