steering wheel shake when i apply my brakes

i have a chevy traverse 2011 and when iam driving around 40 miles per hour start to shake the steering wheel evrytime .I have look on pads and looks good also the rotor looks smooth .

Tire out of balance?
Wheel out of round after hitting a pothole for example?
Brake rotor warped and dragging on the pads each time that high spot or warped area on the rotor comes around. Rotors might look smooth, but be warped.

Bad wheel bearing (usually makes growling noises) or bad half-axle CV joint.
