Fail safe mode

I have a 2000 S type 3.0. Keeps going into fail safe mode. It will go about 5 miles before. I have changed the throttle body and the gas pedal with used one from Ebay. The car use to run like a top. I've tried a hard reset to no avail. Nothing seems to work.
