Gas overflow when filling

We just bought a new Pacifica in December. We have filled the gas tank maybe a dozen times. Twice while filling the automatic shut off on the handle failed and gas sprayed all over my wife's face, hands and clothes. Just wondering if anyone else have had this problem?
