Transmission problems

Hello! I'm new to this forum and would like to hear your answers on my question. I have a 2005 3.4L impala that jerks almost every time it shifts. it is less frequent in the higher gears but there nonetheless. it seems to happen more when the engine is warm but it still happens when its cold. There are no noises or clunks. Sometimes when I accelerate from a stop it jerks. the engine light is on and is throwing an evap code. I'm not sure what is causing the jerking and need help! I have read some posts where it was the transmission solenoids but I'm on a tight budget so I cant throw a bunch of money at it. Is my transmission about to go out?
