2003 Toyota Echo engine very sluggish and noisy.

I have a 2003 Echo with about 97000 miles. All my service/oil changes are up to date.
I have noticed of late that the engine is very sluggish when trying to accelerate to a higher speed, typically anything over 40mph sounds like the engine is struggling.
My technical knowledge is very poor so please excuse me if this sounds wrong but when I accelerate over 40mph, I don't feel the gear shifting for a smoother ride. It feels like it is revving up and just not changing to the next gear(it might be but sounds like a labored process)
Does anyone have any suggestions on what servicing might be necessary for the engine's health?

    It sounds like you arent' shifting into high gear. Why don't you try counting the shifts from a stop? It sound shift 3 times before you reach highway speeds. (it's a 4 speed automatic).

    Are you quite sure you didn't accidentally push a "hold" button on or near the gearshift level? This locks out overdrive, and a little "hold" light might appear on your dash display.

    If the CEL isnt on probably not a problem.
