We don't have the US Bank lease program to check the numbers, but the numbers given look better than the AFS lease program.

The payment calculation seems about right, assuming the sale price is equal to the adjusted CAP cost.

If the $4500 is all for legitimate charges (tax, acquisition fee, DMV/dealer fees), then it looks good. Beware of garbage fees in the $4500.
