2008 Toyota Sienna "Clunk" noise

We have a 2008 Toyota Sienna (approx mileage 142,000). Last night was driving down the road and a clunk was heard in the back and then the car would just rev the RPMs. I am hoping it is not the transmission. Does anyone have any similar problems and what was a solution that worked?

    Do you think maybe the van just downshifted to a lower gear and the clunk and higher revs could be explained by that?
