I have an intermittent shudder/vibration usually over 40 mph - comes and goes.

I have just replaced tires (w/ front end alignment). I have replaced ball joints/sway bars. Original owner. 1999 Honda CRV manual with AWD. I guess I am more concerned about knowing it is safe although aggrevating - has 215k miles. The intermittent thing is what bothers me - it can be fine for two days and then bad for a day. To me this indicates something I am doing, a particular turn or something sets it off. Drove 15 miles to meet my friend for coffee last nite. Perfect travel. Left to drive home, thought the car was gonna implode on the highway. Any clues would be greatly appreciated. I do not take it to a dealer, use a local mechanic - been good and honest to me over the years - told me not to replace the busted timing belt and to move on 50k miles ago..he is a keeper-honest

Any chance it's drivetrain related? Front drive axles? Sensors bad, and the rear axle kicking in, when it shouldn't?

    I had a '98 manual. I loved that car.

Would this happen once or twice a week or once or twice a day, but not for two days - highway driving...seems like it would be consistent, no?

If it's controlled by a failing sensor (ABS sensors control when the rear wheels kick in), then it could be intermittent.

Your description of how violent the reaction is, makes it seem drive-train related to me. If it was an automatic, I'd guess a transmission issue, but that isn't the case here.

If your suspension and tires are good, not sure what else it could be. (not a mechanic)

Let us know when you figure it out. It's cool you kept this car for 22 years.
