When does the $1500 loyalty apply? Is it only if I purchase instead of lease?

Do all these incentives act as cap cost reductions when calculating the monthly lease payment?

No, if you are currently leasing a Mazda, both loyalty incentives will apply - $2150 on top of the $1080 in regular lease incentives.

Yes, they apply like cap reductions.
