I have an offer on a loaner 228xi with ~5000 miles. It's MRSP is $42925 and selling price is 35500. However, they are using ~57% residual because of the miles. Is this correct? Shouldn't this be the other way? Shouldn't the residual be higher for higher mileage car? Would someone please shed light into this?

The residual is what the car is worth at lease end. If it has more miles on it, then it's worth less (lower residual).
What term and mileage for the lease? Generally, the base residual percentage is reduced by $0.25/mile over a brand-new unit.
Just to clarify - the 1500 option allowance can be combined with the 500 loyalty and 1000 USAA for a total of 3000 in incentives - plus whatever discount the dealer is willing to offer? Ask the dealer for the adjusted capitalized cost, and make sure you understand how they got to that number from the selling price.

The number you don't have is the dealer discount from MSRP. On the advertised lease, the assumed discount is $615. This is in addition to the $1500 incentive. The $1500 is used as a CAP cost reduction, so, yes... to get to that payment, you must come up with $3974 at signing. The residual is 61%, not 65%

The advertised lease is just a suggestion. You don't have to make a $2750 Cap cost reduction. You can structure the lease however you want (of course, without the $2750, the monthly price will be higher). It's not even an offer. Dealers aren't required to give that deal.
MSDs are paid upfront, and refundable at lease end. MSD programs are generally only available through luxury makes.
You can use whatever number you want to estimate a lease payment, but you still have to get a dealer to agree to it.
